Please note that our practice area is L39 postcodes ONLY. Unfortunately, we are unable to accept registrations from any other postcode areas.
If you are an existing patient, and you move to an area which has a different postcode, we would ask that you register with a more local GP Practice.

Paper Registration Forms
You can collect registration forms at reception in the surgery, or alternatively, one of the forms can be downloaded to print out and bring in from the link below. When registering, please bring Photo ID and a proof of your address with you. Please note, if you download the form below and bring this to the practice, you will be required to fill out a further registration form which is only available from reception.
All registered patients have a named GP, Dr Darran Harris. However, you are able to consult with any of the clinicians at the practice.

Veteran Friendly GP Accreditation
We are proud to be a Veteran Friendly accredited GP Practice. If you are a prospective patient, please ensure that you inform us during the registration process if you are a military veteran. This will be coded on your medical record and will ensure that all staff involved in your care are aware of your service history.
